چکیده

J.S. Kim, MD S.-H. Jeong, MD Y.-M. Oh, MD Y. Soon Yang, MD S.Y. Kim, MD A 78-year-old man with hypertension and diabetes was referred because of 3 days of diplopia and ophthalmoplegia. Neurologic examination disclosed exotropia of both eyes (wall-eyed) and bilateral internuclear ophthalmoplegia (WEBINO) with impaired convergence (figure, A).1 Vertical saccades and smooth pursuit were also limited, but improved during the oculocephalic maneuver (video). Pupillary and levator function was normal. MRI demonstrated a circumscribed acute infarction in the midline of the mesencephalic tegmentum involving the bilateral medial longitudinal fasciculus (MLF) which are usually supplied by the anteromedial perforators of the posterior cerebral artery (figure, B).2 WEBINO should be differentiated from the exotropia (paralytic pontine exotropia) of oneand-a-half syndrome which denotes unilateral horizontal gaze palsy and internuclear ophthalmoplegia and occurs in the pontine lesion involving the paramedian pontine reticular formation and MLF.3 The dissociated abducting nystagmus, impaired convergence, and supranuclear vertical gaze palsy (video) in our patient support a midbrain lesion damaging the bilateral medial longitudinal fasciculus and pretectum.4

WEBINO and the return of the King's Speech.

We present a 69 year-old man with hypertension who developed the sudden onset of horizontal binocular diplopia and stuttering of speech. On examination, bilateral exotropia (i.e. 'wall-eyed') was observed in the primary position. Attempted horizontal saccades revealed bilateral internuclear ophthalmoplegia; all consistent with the wall-eyed bilateral internuclear ophthalmoplegia (WEBINO) syndro...

Wall-eyed bilateral internuclear ophthalmoplegia in a patient with progressive supranuclear palsy.

Wall-eyed bilateral internuclear ophthalmoplegia (WEBINO) is a rare disorder consisting of a bilateral adduction deficit and primary gaze position exotropia. Associated with bilateral medial longitudinal fasciculus lesions, it has been mostly reported in patients with multiple sclerosis and brainstem stroke. A 72-year-old man with characteristic clinical features of progressive supranuclear pal...

Wall-eyed bilateral inter nuclear ophthalmoplegia with vertical gaze palsy.

A 64-year-old woman, diabetic and hypertensive, presented with sudden onset painless diplopia and alteration of consciousness. She was drowsy with neurological examination revealing exotropia of both eyes in primary position with bilateral internuclear ophthalmoplegia on attempted horizontal gaze (Figure 1a). Pupils were normal bilaterally. Convergence, vertical saccades, pursuits and vestibula...

Wall-eyed bilateral internuclear ophthalmoplegia (Webino syndrome) and myelopathy in pyoderma gangrenosum.

A 35-year-old female with pyoderma gangrenosum developed paraparesis with a sensory level at L1. Three months later she complained of diplopia and was found to have bilateral internuclear ophthalmoplegia with exotropia and no ocular convergence. The term Webino syndrome has been coined to design this set of neuro-ophthalmologic findings. Although it was initially attributed to lesions affecting...

A Case Report of WEBINO Syndrome with Convergence Impairment

A 73-year-old man suddenly presented with alternating exotropia, bilateral medial longitudinal fasciculus (MLF) syndrome, and impaired convergence. This symptom has been known as wall-eyed bilateral internuclear ophthalmoplegia (WEBINO syndrome). Diffusion-weighted images on magnetic resonance imaging showed a smalllocalized lesion in his pontine tegmentum. Based on he had some ischemic risk fa...
